In applying for this training, you are advised that Commonwealth Departments are entitled to collect enrolment and training activity information about you for the purpose of monitoring training quality and reporting on training activity. The information you provide will be accessed to support the ongoing statistical analysis of the Australian vocational training sector. We will not disclose your personal information to any unauthorised persons or organisations. The information collected in this form is required to facilitate your enrolment and will be handled and stored in line with our Privacy Procedures. I understand the terms of this Contract and the refund conditions and confirm that I have been fully advised of the fees, refund conditions and conditions of enrolment and agree to be a student at the RTO. I agree that it is my responsibility to retain a copy of this written agreement as supplied by the RTO and receipts of any payments of tuition fees or non-tuition fees. I agree that under the Data Provision Requirements 2012, the RTO is required to collect personal information about me and to disclose that personal information to the National Centre for Vocational Education Research Ltd (NCVER). My personal information (including the personal information contained on this enrolment form and my training activity data) may be used or disclosed by the RTO for statistical, regulatory and research purposes. The RTO may disclose my personal information for these purposes to third parties, including:

School – if you are a secondary student undertaking VET, including a school-based apprenticeship or traineeship;
Employer – if you are enrolled in training paid by your employer;
Commonwealth and State or Territory government departments and authorised agencies;
NCVER;
Organisations conducting student surveys; and
Researchers.

Personal information disclosed to NCVER may be used or disclosed for the following purposes:

issuing a VET Statement of Attainment or VET Qualification, and populating Authenticated VET Transcripts;
facilitating statistics and research relating to education, including surveys;
understanding how the VET market operates, for policy, workforce planning and consumer information; and
administering VET, including program administration, regulation, monitoring and evaluation.I may receive an NCVER student survey which may be administered by an NCVER employee, agent or third-party contractor. I may opt out of the survey at the time of being contacted.

NCVER will collect, hold, use and disclose my personal information in accordance with the Privacy Act 1988 (Cth), the VET Data Policy and all NCVER policies and protocols (including those published on NCVER’s website at www.ncver.edu.au).

Enrolling with ACIMS

I have been provided information/advice about what is required to successfully complete the training and assessment, and whether this will meet my needs.
I believe that with any required support arrangements in place I have suitable skills and knowledge to achieve what is required to successfully complete this training/assessment.
I give ACIMS permission to locate and confirm my USI number and review my USI VET Transcript using the USI organisational portal, as required by the Australian Government and ASQA regulatory authority.
I have access to the ACIM Solutions Student Information section on their website, including the ACIMS Student Handbook.
I am aware of my rights and responsibilities as a student of ACIMS, as listed in the student handbook.
I know how to make a complaint if I am unhappy about my training or support services.
I agree that if my enrolment is part of an organisational group, or if my organisation is paying for my study, ACIMS may provide my organisation with access to copies of my assessments and certification.

Declaration 5(Required)

Participating in training

I will ensure I have the required resources and equipment as identified at time of enrolment and as asked by my facilitator.
If I know I will miss any scheduled sessions I will advise this as soon as possible, and understand that missing these sessions may impact on my ability to complete required tasks.
If I miss any sessions or required information I will take action to obtain this information as soon as possible.
I have been advised that information for accessing tutorial and Learning support information is available on the website
If I experience any difficulties with the training I will raise this with ACIMS or my facilitator as early as possible so that clarification and support can be provided to me in a timely manner.
If I participate in online/virtual activities, I agree to follow the ACIMS Code of Conduct for E-Learning Programs, which is included in the student handbook.

Declaration 6(Required)

Completing assessment requirements

I will discuss assessment requirements with the facilitator as needed to ensure I understand what is expected of me.
I will manage my work load to complete required assessment tasks within the agreed time frames.
If I need to vary an assessment deadline I will contact ACIMS and arrange this before the due date.
I understand I may be required to undertake independent research and show initiative to find answers to some assessment questions.
I will provide my written assessments in a typed word document, unless other arrangements have been agreed to by ACIMS.
If I use research and documents created by others as part of my assessment answers I will acknowledge this in my work.
I understand that work submitted must be authentic (my own work). If I collaborate as part of the training/assessment activities, I will still use my own words in the answers provided and give due acknowledgement to any contributions made by a wider group. I am aware that plagiarism is addressed in the Student Handbook.
